Planning for Your Visit
Arrive 10-15 minutes early to fill out paperwork, or you can print off the two forms below (Patient Form #1 and #2) and bring them to your appointment.
If you would like us to direct bill your insurance company, bring your insurance information. We are not able to tell you what coverage you have for chiropractic care. You have to call your insurance company directly or obtain this information online.

Your First Appointment
When you schedule an appointment with us, the time has been reserved for you. If you are unable to keep your appointment, please contact us as soon as possible to reschedule.

During your first evaluation, you will consult with your doctor about your symptoms, undergo an orthopedic and neurological examination, and discuss treatment options. You will then learn techniques such as stretching and application of heat to do at home. At the end of your visit, you will schedule a follow-up appointment if necessary.
Insurance and Payment Information
We can direct bill with most insurance companies. We also treat WSIB cases and motor vehicle accident patients and work with them for direct billing. If you are unsure how your insurance works, or if you have questions about your chiropractic coverage, give us a call.
Payment is due at the time services are rendered. This includes co-payments and deductibles. For your convenience, we accept Interac, Visa, and MasterCard.
